Service Delivery Standards

The Service Delivery Standards contains information on required implementation arrangements for all Outreach programs and are to be read in conjunction with the respective program’s Agreements and Schedules.

Standard Grant Agreement

The Standard Grant Agreement sets out the standard terms and conditions applicable to all Health Access Outreach Programs funded by the Australian Government Department of Health, Disability and Ageing and administered by RDN Health.

The Agreement must be read together with the Schedule.

Contracts

Contract templates are provided to support subcontractors developing contracts for visiting health professionals and are provided as a guide only.

Activity Reports

Activity Reports are to be completed using the program templates below and submitted to RDN Health within 14 days of delivering an Outreach clinic and seven days prior to payments being processed.
