Collaborative Care for Communities

The Collaborative Care Program is a community-centred approach to addressing health care challenges in regional New South Wales (NSW). These communities have smaller populations that are more dispersed, a lack of transport options and fewer locally available services.

The Program is coordinated by RDN Health in partnership with the NSW Ministry of Health, local health providers and community organisations.

The Program works with local health professionals and communities to create a healthcare access model that fits their needs. It does this by bringing stakeholders together to develop shared priorities and solutions that make sense for each community.
