RDN Health’s Community Solutions unit empowers communities with the information, connections and support they need to make informed decisions, advocate effectively, and address their health and social care needs.
The Community Solutions team brings together insight, stakeholder relationships, data and evidence to identify local priorities and develop coordinated responses that address health access challenges. By connecting people, organisations and resources, we help create practical, community-informed solutions that strengthen local systems.
Our approach focuses on understanding what matters most to communities, identifying gaps and opportunities and working collaboratively to implement sustainable responses.
Community Solutions is a key pillar of RDN Health's support for under-resourced communities, encompassing elements of town-based planning and stakeholder engagement, reinforcing its commitment to equitable health access.
What we do:
Community Solutions supports health access improvement through interconnected areas of work:
RDN Health’s place-based approach works alongside communities to identify priorities, build partnerships and develop locally relevant solutions. The Community Solutions team facilitates stakeholder engagement, supports co-design processes and helps align organisations around shared goals.
This work may include community consultations, health access action planning, stakeholder coordination and support for collective local responses.
Local government plays an important role in influencing health access within communities. Community Solutions works with councils to better understand health access challenges, support planning and identify opportunities for local action.
RDN Health provides a flexible, place-based approach that supports communities to address identified health priorities. By working alongside local stakeholders, services and health professionals, Community Solutions supports the design and delivery of targeted initiatives that bring opportunities directly to remote, rural and regional communities and focus on strengthening the existing workforce to reduce reliance on external services.
Health Priority Focus Area: Diabetes
Diabetes is a demonstrated focus area for this approach and remains a current area of activity. This work builds on the work achieved via NSW Ministry of Health’s Collaborative Commissioning Program – Care Partnership – Diabetes (CP-D). The program was delivered in collaboration with Western NSW Primary Health Network, Western NSW Local Health District and Far West NSW Local Health District.
Between 2022 – 2026 CP-D significantly strengthened diabetes care capacity across Western and Far West NSW through a focus on locally led initiatives, workforce sustainability and partnerships.
Building on the success of the program, Community Solutions is exploring opportunities to expand the adapt key workforce capability and service access initiatives, including:
- Workforce Capability Wrap-Around Supports
- Access to education, training and upskilling
- Case management support through post-grad qualifications
- Mentorship pathways
- Localised networking
- Workshops and Training opportunities – focused on place-based and in-person opportunities for rural, regional and remote workforce
- Locally-led Diabetes Care Initiatives focused on improving patient access to care
